Background: The German registry for cataract surgery was established in 2014. The main aim of this registry was to improve cataract surgery outcomes. Aim: The aim of this project is to offer a tool for benchmarking through the establishment of a reference database in Germany where surgeons could record and analyze their own outcomes. Methods: Data were collected between January 2018 and December 2018. The analyzed data included parameters of surgical technique, implanted intraocular lens (IOL), refractive and visual outcomes. Results: During the year of 2018, the German registry included 10,035 lens exchange surgeries and the complete follow-up was available for 9882 lens extractions. Approximately one third of the patients were between 76 and 80 years old and 60.6% of the cases had a best-corrected distance visual acuity (CDVA) before surgery of 0.5 or better. Parabulbar anesthesia was reported in 60.4% of the cases and phacoemulsification with implantation of a posterior chamber IOL was the chosen method in 92.2% of the cases. The power of the implanted IOL was between 21.0 - 22.9D in 29.4% of eyes. Surgical complications were reported in only 413 cases. After surgery, a CDVA of 0.5 or better was achieved in 90.8% of the cases. Most of the operated patients (64%) had a residual refractive error within ±0.5D (95% confidence interval 63.2 - 65.1). Purpose: Biometry measurement of pre-operative cataract patients plays a major role in calculating intraocular lens power. This study aimed to review and determine the distribution of biometry components, such as axial length (AL), anterior chamber depth (ACD), and lens thickness (LT) of pre-operative cataract patient in Indonesian population. Methods: A cross-sectional study data were obtained from 2015-2018 cataract surgery at Prima Husada Citra Hospital, Surabaya, East Java, Indonesia. A total of 1295 eyes with cataract was included in the study based on their medical record. We analyzed the data using descriptive analysis and correlated each variable using the Spearman’s Rho analysis. Results: Mean AL, LT and ACD was 23.81 ± 1.46 mm, 4.49 ± 0.55 mm, 3.25 0.70 mm respectively. Male has longer AL and deeper ACD with (p < 0.001), while LT was found thicker in female (p = 0.005). The increase of AL was accompanied by ACD (r = 0.457; p < 0.001) and a thinner LT (r = −0.101; p < 0.001). Conclusions: LT is increased with age while AL and ACD are shortened with age. There was a positive correlation between AL and ACD, but an inverse correlation between AL and LT....
Background: Macular hole is a known cause of central visual defect with varying etiologies in different parts of the world. It was considered an untreatable condition until 1991 when varying treatment techniques began to emerge with resultant restoration of central vision. This study was carried out to evaluate the prevalence and pattern of Macular hole in a tertiary eye centre in Nigeria. Method: This is a prospective hospital-based study over a 5 year period spanning from November 2013 to October 2018. All patients with macular hole seen within this period were included in the study. Relevant data were recorded with a structured questionnaire and analysed with SPSS version 20. Results: There were 46 eyes of 42 patients which constituted about 0.5% of all new patients seen over the study period. Male to female ratio was 1:1.3 with age ranged between 15 and 91years (mean = 61.8 ± 16.5 years). The majority of the patients (95.7%) had Idiopathic macular hole. Half of the patients (54.8%) had visual acuity less than 3/60 in the affected eye at the time of presentation. The majority (95.7%) of the patients did not accept the recommended surgical treatment option because of financial incapacitation. Conclusion: Macular hole is a disease of marked visual consequence. Available treatment option is still not within the reach of the majority because of the high cost....
Background: Acquired See-saw Nystagmus (SSN) is a rare form of nystagmus characterized by elevation and intorsion of one eye with synchronous depression and intorsion of the contralateral eye in the first half cycle, followed by a reversal in the direction of the movements during the next half cycle. We herein report a case of a 47-year-old woman with a 3-year history of constant diplopia as a consequence of multiple neurosurgical interventions due to hemorrhage from a cavernous angioma located in the subthalamic region. She also had a history of major depressive disorder and ulcerative colitis. The patient underwent a surgical intervention with a 5 mm bilateral recession of the superior and inferior rectus muscles. The aim of the HYLAN M study was to investigate if symptoms and/or signs of patients suffering from severe dry eye disease (DED) can be improved by substituting individually optimized artificial tear therapy by high molecular weight hyaluronan (HMWHA) eye drops. In this international, multicenter study, patients with symptoms of at least ocular surface disease index (OSDI) 33 and corneal fluorescein staining (CFS) of at least Oxford grade 3 were included. A total of 84 per-protocol patients were randomized in two study arms. The control group continued to use their individual optimum artificial tears over the study period of eight weeks; in the verum group, the artificial tears were substituted by eye drops containing 0.15% HMWHA. At the week 8 visit, the average OSDI of the verum group had improved by 13.5 as compared to the control group (p = 0.001). The best corrected visual acuity (BCVA) had improved by 0.04 logMAR (p = 0.033). CFS, tear film break-up time (TBUT), Schirmer I, lid wiper epitheliopathy (LWE), mucocutaneous junction (Yamaguchi score), and tear osmolarity were not significantly different between the verum and control groups (p > 0.050). We conclude that for most patients with severe DED, 0.15% HMWHA eye drops provide excellent improvement of symptoms without impairment of dry eye signs....
